1) All blank field lines in yellow MUST have an answer (even in the bottom section "Parent/Guardian Waiver for Minors." If this form is being filled out for an adult only, please fill out the top section and (instead of skipping the answers on the bottom minor section) place "N/A" in each and all of the minor section fields instead of leaving them blank or the form will not send.
2) Once signed, check your e-mail box to confirm your e-mail.
Waiver Form-
Form must be filled out for each individual attending, both children and adults. A separate form must be sent for each adult/child attending.
